Skin Tears and Treatment through Wound Be Gone

The skin is the largest organ of the body and often the most forgotten. Skin is exposed daily to numerous environmental risks of irritants and chemicals, as well as to physical and mechanical injury. How are skin tears caused?



When skin tears are reported to healthcare professionals, the cause of the injury often is not known. It has also been concluded that the key times when skin tears occur are during peak activity hours, from 6:00 to 11:00 am and from 3:00 to 9:00 pm. Skin tears result from shearing, friction, or blunt trauma that causes separation of skin layers. The subsequent wounds are partial- or full-thickness, depending on the degree of tissue damage. Subtle skin changes associated with aging increase the risk of skin tear development and interfere with the healing of the skin tear. Aging skin undergoes a process in which it experiences dermal and subcutaneous tissue loss, epidermal thinning, and serum composition changes, which cause decreased skin surface moisture. The skin's elasticity and tensile strength decrease as these other changes occur. Dehydration, poor nutrition, cognitive impairment, altered mobility, and decreased sensation can also increase skin tear risk.



Skin tears are a painful, yet preventable problem. In the United States, reports show that 1.5 million skin tears occur each year in institutionalized adults. These types of wounds pose a potentially serious and painful problem for the older adult population, who are at a higher risk for skin tears. Such wounds can cause stress to both patients and their families. The result of the physical changes associated with aging and coexisting illnesses can be very difficult to manage. Thus, it is imperative that healthcare professionals identify those at risk for skin tears and establish plans of care that will address prevention, as well as evidence-based management of these wounds. Compared with more extensive and costly chronic ulcers, skin tears are often seen as minor, inconsequential wounds. But in reality, these wounds are painful and can lead to potential complications if not treated appropriately.



How can skin tears be prevented?



Following should be considered if patient is at risk of having a skin tear:



• Educate staff on the importance of carefully handling elderly patients with

frail skin. Any harsh movement or pulling can create a skin tear.

• Encourage proper positioning, turning, lifting, and transferring techniques

to prevent friction or shear. A lift sheet should be used to move and

turn patients. If the patient is being cared for at home, make sure the

home health care assistants and the patient's family caregivers understand

these techniques.

• Provide padding to bed rails, wheelchair arm and leg supports, and any

other equipment that may be utilized to protect the patient from

accidentally bumping into a hard surface.

• Use pillows and blankets to support arms and legs.

• Recommend that patients wear long sleeves and pants for added protection.

• Use paper tape or a non-adherent dressing on frail skin and gently remove it.

Or use stockinet, gauze wrap, or any other similar type of wrap instead of

tape to secure dressings and drains.

• Apply a moisturizing agent to dry skin to keep it adequately hydrated. Creams

and topical gels are better than lotions.

• Provide a well-lit environment to minimize the risk of patients bumping

into equipment or furniture.

Studio Lighting Equipment - How to Get Professional Results on a Budget

There is a lot to be said about having the chance to shoot in a studio filled with expensive lights and equipment perfect for creating the perfect photo. Unfortunately, most people who love photography as a hobby, don't have the budget to spend money on a professional lighting equipment setup. In fact, if you enjoy photo as a hobby but not as a profession, there's really no reason to spend so much on something that you will use so little. Here are some ways that you can get studio-quality images simply by using the light you have around you.



Home-made Reflectors



One of the keys to studio lighting is "bouncing" the light off of reflectors or umbrellas to create full, soft, and even light. Though this is hard to do without a few different light sources, you can mimic the effect by making your own reflectors to use when you shoot. Lots of things will work to help soften the shadows that are created when you're only using a single light source such as the sun. For example, you can use a white sheet, a large piece of poster board or even a large piece of Styrofoam.



Use Backdrops Creatively



You may not be able to have a large selection of photo backdrops like in a professional studio, but that doesn't mean you can't have fun creating one of your own. You can use bed sheets for basic colors and then if you want to get creative you can always use paint to create a pattern or add color. The best thing to do is hang your backdrops on the opposite side of a big window or a large bright opening (like a garage door opening) and it will be like you're using a fill light pointed directly at the subject.



Get Out the Paint Brush



Whenever you go to shoot, always remember that all surfaces either reflect or absorb light so keep this in mind especially when designing a room for taking pictures. If having a lot of lights in the room isn't an option, then you should consider painting the walls and floor white. The white walls will help to bounce the light around for a more even tone.



Control the Flash



A direct flash will often make the subject have harsh shadows or look shallow but if used correctly it can be a great lighting tool. Whenever possible, look to bounce it off surfaces around your subject and only use it to fill in the shadows from the front. Think about it this way - if the image you are building looks like you won't need a flash, that's a perfect time to use one for additional fill.



Watch Your Time



Light looks different at different times of day. If you use your homemade reflectors to soften the shadows and use your backdrop facing towards the early morning light, you'll be happily surprised at the great results you'll get. Try taking photos at different times of day to see how the difference in light impacts the way things turn out, then book your studio sessions in for the times in which you'll have the best light possible.



It's important to remember that studio lights, for the most part, were created to simulate the light that we have in our every day lives. If you know how to harness this light and make it work for you, there's absolutely no reason that you shouldn't be able to generate images that compete with those created in the most expensive studio in town. If you have the desire and creativity to do it, you can create your own makeshift studio with a little bit of effort and ingenuity. At the end of the day, the only important thing is the result - the method is secondary.

How To Really Save Planet Earth: A Test About Al Gore


If you really want to bring the Earth back from the brink of disaster, who should you be listening to?

A. Al Gore of "Live Earth" and "An Inconvenient Truth"
B. Diane Sawyer of ABC's "Seven Ways to Help Save the World"
C. Michael Brune of "Rain Forest Action Network"
D. Conservative commentator Glenn Beck of "CNN Headline News"

"An Inconvenient Truth" left out the most inconvenient truth of all, and the Live Earth concerts were an environmental joke (and the music was really bad, too). Ms. Sawyer's suggestion to cut back on toilet paper is a bit of poo itself. Then, with a golden opportunity to enlighten and empower Americans, Michael Brune of the Rain Forest Action Network only highlighted the problem with even our "environmental" groups. Therefore, and by way of deductive reasoning, the answer must be Â… and is Â… (drum roll, please) Â… Glenn Beck. Yep, that's right. Glenn Beck, the ultra-conservative, speaks out on CNN Headline News while interviewing Michael Brune and tells people how to really save the environment.

In speaking about the Live Earth concerts, Glenn said, "There's nowhere in here about vegetarianism or anything like that, that would make a real impact on the environment ... It is animal gases that produce more CO2 than any of the cars we're driving."

To which Michael Brune disagreed. Michael? HUH? Earth to Michael Â… Come in, Michael.

The fact is, a HUGE portion of our environmental problems come from eating meat and dairy (and from producing meat and dairy), and Glenn Beck, of all people, hit the nail square on the head. Global warming, rain forest destruction, coral reefs dying off, topsoil erosion, poisoned water and polluted air are all a result of animal agriculture, and everybody – including our "environmental" groups – refuses to connect these dots. Thanks, Glenn, for doing what Al Gore, Diane Sawyer and even Michael Brune have refused to do.

Kids, and all Americans, need to know that they are the empowered ones. This is not a political or governmental issue. WE, each of us, has a hand in global warming, and, therefore, its cure. If we stop eating animals and their secretions, we will stop killing the planet. It is as simple as that. Concerts galore, one million TV shows on the "Top Seven Ways ..." and pseudo-environmentalists be damned. Without veganism for all people, nothing else can possibly save our doomed planet. The great news is, if we each switch to eating a vegan diet, we will no longer have to "save the planet." Veganism allows the planet to save itself.

At first glance, this topic may not look like anything new to you; nor to me. After all, it is a routine fact that today hire many corporations hire business consultants all the time. I'm guessing, as we speak someone out there has hired a consultant for an outside perspective of a specific business matter.



Over the years, an endless number of companies have outsourced this task to specialists. But in recent times, an important change in the industry has been the spin-off or separation of the consulting units of large diversified firms. Although an initially expensive proposition, the long term benefits of setting up such a group eventually out-weighs the cost. These corporations have set up their own internal consulting groups, hiring internal management consultants either from within the corporation or from external firms employees. Many corporations have internal groups of as many as 25 to 30 full-time consultants catering to functional areas such as organizational development, process management, information technology, design services, training, and development.



But before a corporation steps into the task of creating its own internal management consulting group, they need to be conscious of and consistent with how this internal cost is accounted for on both a project and organizational level. At the end of the day, it needs to be cost effectiveness as well as provide significant value to the organization's operations. On one hand, these internal groups are capable of evaluating the engagement on projects in sync with the corporation's strategic and tactical objectives. Besides the low degree of dichotomy in terms of priorities (They put the company objectives first) and less ramp up time on a project due to its familiarity with the corporation, internal consulting groups also ensure that vital corporate information is not leaked out and is kept private. Moreover, the time and material cost of internal consultants is significantly less than external consultants operating in the same capacity. But the crucial difference lies in the fact that, unlike external consultants, the job of the internal management consulting team does not end with the mere presentation of the business solution. They are equally are involved in the monitoring and implementation of their recommendations.



However, there is always a flip side to everything good. Defragging Drive May Work Best When Starting in Safe Mode


The following is a question posed to Array Systems Inc. (http://www.arraysystems.com/), an IT outsourcing and Computer Network Support company serving Los Angeles:

"When attempting to defrag the C drive on my 333 E Machines PC using Windows 98 OS 6.1 edition, the first message I get is “Reading Drive Information. Defrag begins but will not go beyond 10 percent completion. Next a blank screen appears, th. This sequence recurs every 3 to 4 minutes."

What you’re experiencing is conflict/contention with another open program. When the defrag utility runs, it copies information (data, programs or files) from one area of your hard disk to a clean area. If a program is open or a file is being accessed by another program, the defrag utility will not copy it to the clean area. These programs probably are part of your startup process. For example, my startup process automatically opens my virus scan program so that when I get on the Internet I’m protected. If I want to run my defrag utility, I need to start my computer in safe mode.

Safe mode is a scaled down version of your Windows operating system and will bypass your startup process so that programs that are automatically turned on will not be initiated. You’ll notice a difference right away because your Windows desktop will look different and your icons will appear larger than normal. This is because the default screen resolution is larger than your normal settings.

For those who aren’t familiar with the defrag utility, let me take a few minutes to explain what what exactly it is. There are two programs you should run on a regular basis, which should be part of your disk maintenance program. These programs are disk defragmenter and scandisk.

I run my disk defrag once a quarter and my scandisk once a month. The disk defrag utility cleans up your hard disk to make it more efficient. Let me explain how your hard disk works and why it needs to be cleaned up periodically.

I’m writing my column using MS Word. When I open the file for my current column, I like to save it after 5-10 minutes of typing. When I save it, the Word document can’t be saved over the same spot on my hard disk. Therefore, a new spot on the hard disk must be found and the pointer (index) must be updated to reflect the new location. Sometimes, if the file is very large (novel or screen play), there isn’t enough clean space in one location so the file is broken up into several locations with several pointers. The old space is now unusable until the next defrag utility is run.

Sometimes, you’ll notice that your computer takes a long time to save a document because it is hunting around for some clean space. This is probably the No. 1 problem associated with computer performance. The defrag utility starts by copying all the files from the beginning of your hard disk to another location. Then it cleans the disk area and copies the current files back. It continues with each area/sector or your hard disk until it cleans the entire disk. This process can take up to eight hours, depending upon the size of your hard disk and the computer’s speed.

The next utility is called the scandisk. It resembles the defrag utility, but it updates only the pointer/index files. Every time a file is updated, the new location is updated in your index file. These files are the most active files on your computer because it tracks the location of everything. The scandisk also eliminates any corruption problems that have occurred when shutting down your computer improperly or when your computer hangs.

A well-maintained index also will increase the speed and efficiency of your computer.

When automobiles first came out, the only car safety feature they had was a brake. No safety glass, no seat belts, no air bags. Driving back then was a sure-fire hazard that could get you killed. Thankfully, those times are over.

However, vehicle accidents continue to happen, often with devastating results. Car safety devices such as seat belts are commonplace today, but they are not always used. Drivers in these states must buckle up, or they will be fined.
